IRS Form 1099-R reports all of the distributions you've taken from your IRA or 401k retirement accounts during a calendar year, whether or not those amounts are taxable. All distributions must be reported and a taxable amount calculated which is then entered on your Form 1040 federal tax return and your state return. Fortunately, filing out your 1040 usually completes much of the work required for the state tax return. Your 1099-R form will provide the remaining information that you'll need to file your state taxes correctly.

Take a distribution. To receive a 1099-R, you must withdraw money from one or more of your retirement accounts. This withdrawal will be reported to the Internal Revenue Service as a distribution. You will receive a 1099-R for each retirement account from which you've withdrawn funds during the year.
Gather all of your 1099-R forms. You cannot file your taxes until you have received a 1099-R for each account from which you made a withdrawal. Consult with your state tax board to determine any other filing requirements specific to your state.
